My mom had lived happily and independently for many years, but lately she’d become increasingly forgetful, had a bad fall and found it difficult to keep up with all the household chores. We both agreed it was time to think about a move to a community that offers her more assistance. But where to begin? What’s available? Can we afford it? Thankfully, Partners for Transitions was able to guide us through the whole process. Now mom is settled in a great place---and once again enjoying her life. I’m relieved.
The Alternate Care Program provides assistance in exploring which facility options best match your needs, wishes, and financial resources.
• Assists with long-term care planning, which might include staying at home with assistance or moving to a facility.
• Maintains an updated list of facilities available to you if you are temporarily or permanently unable to live independently.
• Orchestrates your move.
