Sandy Larson, Coordinator of End of Life Program
Sandy retired after working nearly 30 years for Hospice of the Central Coast as the medical librarian and resource specialist. Her work with patients and families as well as the medical community brings a wealth of experience and understanding to Partners for Transitions. Her current role as coordinator for the End-of-Life Program is a natural and welcome sequel to earlier experiences. She also works with Delana Schneider in their business, HearthStone, Organizing with Heart, a service which helps people de-clutter and “right-size” their homes. She lives with her husband in Monterey; the couple has two sons. |

Bob Petty, PhD., Boomer Transition Advisor
With over ten years as manager of the Medicare advising program and Community Educator for the Alliance on Aging, Bob is known for his knowledge and service helping seniors and their families. He has given over 300 workshops, seminars, and classes throughout Monterey County.
At Partners for Transitions Bob specializes in “Planning For Your 60s.” He informs and advises emerging seniors, the “Boomers”, with decisions regarding issues such as Medicare, long-term care, and Social Security.
Outside of this work Bob serves on the Advisory Council for the Area Agency on Aging and represents his district in the Assembly of the California Senior Legislature. In 2005 he was a California delegate to the White House Conference on Aging. His other community activities include volunteering for the SPCA, as a docent at Point Lobos, on the Monterey City Historic Preservation Commission, and on the board of Friends of the Monterey Public Library. |

Tammy Christensen, Administrative Assistant
Tammy’s role as Office Assistant for PFT combines her experience working in the Investment Retirement Industry and caring for family members who have had serious injuries and illnesses. She believes in the value of partnerships that honor each individuals dignity and, in her experience, the “priceless gift” that one on one support provides in time of crisis. She is the first “voice “ you will hear when you call our office!
Her greatest loves in life are her four children, two boys and two girls. She was a stay at home mom for almost 10 years and enjoyed being involved in school activities, fundraisers and her kids’ many sporting events. She enjoys cooking, baking, gardening and walking her dog. |

Donna George, RN, RN Consultant for PFT
Donna has been a registered nurse for over 40 years with experience in medical surgical nursing, public health nursing and coronary care. After moving to the area 12 years ago, she worked for Carmel Valley Manor as Director of Staff Development and charge nurse in the skilled nursing facility. Currently she works part time for Hospice of the Central Coast as a clinical instructor. Her nursing background, education and experience provide a great support to the team as well as the clients and families working with Partners for Transitions. She has been a member of the Big Sur International Marathon board for the past 10 years. She lives in Big Sur with her husband and has 3 grown children. |

Melissa Granstrom, RN, BSN, RN Consultant for PFT
Melissa is passionate about patient healthcare,education and advocacy. She is a charge RN at Community Hospital of the Monterey Peninsula and is a valuable medical consultant for PFT. Her trained and skilled assessments insure that our clients needs are addressed as their health changes. She is a vital link in our team that supports our clients thru life transitions. |

Kate Haggerty, Pop-In Program
Kate worked for many years in the theatre and then producing festivals. Her main job was logistics and organizing volunteers. She loved working with the volunteers because they were people that wanted to give back to the community. Kate helped her mother prepare for death the last 3 years of her life. The challenges of being the main caregiver during those years revealed to Kate that being present and compassionate are valuable skills that she can offer to people at a time when they are the most vulnerable. Kate has since volunteered at the Red Cross, food banks, and hospitals.
Kate joined the Partners For Transitions team last year as a Pop-In. She finds this committed work to be a perfect match for her life experience and skills as a compassionate caregiver.

Jenny Miller, Pop-In Program
Jenny Miller lives in Carmel Valley with her husband and their daughter. She is a graduate of Carmel High School and UC Santa Cruz. Jenny joined the Partners for Transitions team in 2009 as a "Pop-In." Prior to joining PFT Jenny worked as a personal assistant for 2 senior clients. She found that she enjoyed working with seniors and chose to continue through the "team approach" of PFT. Jenny appreciates the opportunity that each client offers for a unique relationship grown out of caring, trust and companionship. |

Robin Rote, Pop-In Program
Robin recently retired after 45 years of teaching on the Monterey Peninsula. She taught K-12 with a focus on Special Education, which required devising instructional strategies for both academics and social needs for her students.
Robin's husband, Dr. James W. Rote, suffered from MS for 18 years. Robin was his primary caregiver through the various stages of the disease. She is widely experienced as a a patient advocate with the administration, and the care aids who worked for Jim at home, then assisted living facilities and finally the convalescent hospital. This tremendous experience gives her insight into the needs of the Pop-In clients and their families.
Patience, compassion, and a huge giving heart are qualities she brings to her job with a joyous manner. |

Mary Schrady, Pop-In Program
Mary's education and life experience have been in the healthcare field. Mary has worked for Partners For Transitions as a Pop-In since October 2008, deriving a great sense of fulllfimment from her work with clients.
As a volunteer and Board Member for Planned Parenthood, Volunteers in Action, Junior League of Monterey County, along with the Monterey Peninsula Chamber of Commerce and the Naval Postgraduate School Staff & Faculty Club, she has made lifelong friends with like-minded caring people.
Mary and her husband, a retired Professor at the Naval Postgraduate School, are Peninsula residents since 1965. They now enjoy traveling together in their free time.
Mary is the mother of three adult sons and her hobbies are her four active and lovely grandchildren. |

Kelly Shannon, Pop-In Program
Kelly has a diversified business background. She owned and operated Private Design Schools in California. She was an Interior/Architectural designer in California,Hawaii and Italy. She has lived in Carmel Valley for the last twenty years, where she raised her daughter while owning and managing a retail store and wholesale jewelry business.Kelly felt privileged to take care of two Aunts, one Uncle and her mother during the latter years of their lives. She navigated many medical issues,different levels of dementia including Alzheimer's and her mother going blind in the last few years of her life.
Her many experiences with hospitals,convalescent homes,board and care homes, assisted living,hospice; both at home and facilities has given her a broad understanding of what is needed to be available to both her private and Partners For Transitions clients for the last three years. |

Beth Stratton, Pop-In Program
Beth has lived in Carmel Valley for over forty years. Because she was fortunate to be able to stay home with their two children, she and her husband also have fostered a number of children and hosted many exchange students.
Before joining PFT in 2011 as a Pop-In, Beth gained experience in caregiving for elderly individuals and helping families cope with progressive diseases. In addition to working one-on-one with individual clients, Beth also enjoys gardening, traveling, and staging homes for realtors

Jill Verhaak, Pop-In Program
Jill is a retired Monterey County business owner and lives in Monterey. For the past 5 years, she has been actively providing personal support and assistance for two dear friends and their families, by running errands, driving to doctor appointments, shopping and helping with end-of-life arrangements. Jill believes that her work as a Pop-In for Partners For Transitions fills her need to be of service to individuals and their loved ones in a professional setting with the support of a team. |
