• Provide the appropriate document

• Work with you in choosing someone you trust to serve as your Agent/Alternate Agent

• Encourage you to discuss your wishes with physicians, family, friends and agent(s)

• Send copies to designated persons, upon completion of the form

• Provide a laminated I.D. card for your wallet

• Check in with you, annually, to review/update your form

• Serve, when appropriate, as Agent/Alternate Agent on your behalf
